Il rapporte que la couronne provisoire est désormais mobile et instable.\nÉtant donné que nous sommes actuellement à **3 semaines post-opératoires**, l’implant se trouve dans une phase critique de cicatrisation où la stabilité mécanique primaire diminue alors que la stabilité biologique n'est pas encore pleinement établie. Je vous serais extrêmement reconnaissant de bien vouloir évaluer si cette mobilité est purement prothétique (liée aux composants provisoires) ou si le corps de l'implant a perdu son intégration.\n#### **Protocole Clinique Recherché**\n#### **Étape 1 : Diagnostic et Accès**\n 1. Veuillez accéder délicatement à la vis de la couronne provisoire par le puits d'accès palatin (cingulum).\n 2. À l'aide d'un **tournevis hexagonal standard Osstem**, tentez de dévisser très délicatement la vis prothétique.\n 3. **Évaluation :** Déterminez si la mobilité est due uniquement à un desserrage/rupture de la vis, ou si le corps de l'implant lui-même est mobile à l'intérieur de l'alvéole.\n#### **Étape 2 : Conduite à Tenir Selon les Constatations**\nVeuillez suivre l'une des trois voies suivantes en fonction de vos observations cliniques :\n#### **Option A : L'implant est parfaitement stable (Simple desserrage de la vis ou de la provisoire)**\nSi le corps de l'implant ne présente absolument aucune mobilité :\n * **Ne pas revisser la couronne provisoire** sur l'implant. Je souhaite éviter toute charge dynamique ou risque supplémentaire sur la fixture.\n * **Action :** Mettez en place une **vis de couverture Osstem** (ou une vis compatible avec hexagone interne). Serrez-la très délicatement pour ne pas perturber l'interface os-implant.\n * **Solution esthétique :** Enfouissez complètement l'implant sous la muqueuse. Réalisez en urgence une **prothèse partielle amovible (appareil résine)** ou une **gouttière thermoformée avec une dent à tenon (pontic)**.\n * *Crucial :* Veillez à ce que l'intrados de l'appareil ou de la gouttière soit totalement déchargé (meulé) en regard du site de la 11 afin qu'il n'y ait **absolument aucune pression ni interférence** sur la vis de couverture ou la muqueuse.\n#### **Option B : L'implant est mobile mais correctement positionné (Pas de fracture osseuse)**\nSi le corps de l'implant bouge légèrement mais semble structurellement sain, et qu'un examen CBCT (si disponible) confirme qu'il reste dans une position 3D acceptable :\n * **Action :** Si nécessaire, réalignez légèrement le corps de l'implant manuellement pour que le profil d'émergence corresponde au cingulum de la 11.\n * **Protection :** Placez une vis de couverture, enfouissez complètement l'implant, et laissez-le totalement tranquille pour cicatriser/se réintégrer pendant un minimum de 3 mois.\n * **Solution esthétique :** Réalisez un appareil provisoire amovible en urgence, en veillant à ce qu'il soit parfaitement déchargé au niveau du site chirurgical.\n#### **Option C : L'implant est très mobile, mal positionné, ou la table vestibulaire est fracturée**\nSi l'implant a perdu toute stabilité, s'est sévèrement déplacé, ou si vous suspectez une fracture de la paroi osseuse vestibulaire :\n * **Action :** Veuillez **déposer (retirer) délicatement le corps de l'implant**.\n * **Suivi :** Laissez le site cicatriser proprement. Cela nous permettra de planifier une pose d'implant différée simple à l'avenir.\n * **Solution esthétique :** Réalisez un appareil provisoire amovible en urgence pour des raisons esthétiques évidentes.\nCompte tenu du fait qu'il s'agit d'une incisive centrale, le patient est particulièrement anxieux concernant l'aspect esthétique.
